腦膜瘤以耳息肉表現－病例報告
Meningioma Manifesting as Aural Polyp － Case Report
摘要
腦膜瘤約佔所有顱內腫瘤的13～18%，顱外腦膜瘤則屬罕見。顳骨可被後或中顱窩之腦膜瘤所侵犯，亦可長原發腦膜瘤。本科經歷一病例，為42歲女性，因自幼兩側耳痛及耳漏，於1983年7月在本科接受兩側乳突根除術，術後情況良好。後來發生頭痛及吞嚥困難等症狀，於1986年因為右側顱底腫瘤，曾在他院接受手術治療，病理報告證實為腦膜瘤，術後情況順利。病人於1989年開始發生右側耳痛及耳漏，於1990年在局部檢查下發現右耳有息肉樣腫塊發生，病人於1991年12月接受手術，病理報告為復發性腦膜瘤，高解析度電腦斷層掃描發現有右側復發性顱內及顱外腦膜瘤，因此照會腦神經外科作進一步的評估及治療。

Meningiomas constitute 13-18 % of all intracranial tumors. In contrast, extracranial meningiomas are very uncommon. The temporal bone can be involved either by spread from the posterior or middle cranial fossa; or, rarely, it can be the seat of a primary extracranial meningioma. We experienced a 42-year-old female, who had been suffering from bilateral otalgia and otorrhea. Bilateral radical mastoidectomy was performed in July 1983. In 1986, skull base tumor was diagnosed and craniotomy was performed. The pathology revealed meningioma. In 1990, right aural polyp was noted and revised mastoidectomy was performed in January 1992. The pathology revealed recurrent meningioma. High resolution CT showed recurrent meningioma with both intracranial and extracranial involvement on the right side.
