正常聽耳變頻耳聲傳射之特性

Characteristics of Distortion Product Otoacoustic Emissions in Normal Hearing Ears

摘要


為探討變頻耳聲傳射在正常聽耳的特性,以建立臨床運用的基本資料,本研究以26側正常聽耳,使用相同強度的兩刺激音誘發記錄2f1-f2頻率之變頻耳聲傳射(DPOAE),以不同刺激音量求得自0.5至8 kHz,每1/2八度音頻率的變頻耳聲傳射圖及輸入/輸出函數曲線,並探討變頻耳聲傳射閾值與純音聽力閾值的相關性。我們在25側正常聽耳中可靠地記錄2f1-f2變頻耳聲傳射,佔96.2%,各頻率之閾值分布為15-60 dB SPL ,平均之變頻耳壁傳射圖呈雙高原型,其中包含1 kHz 以下的記錄。單獨記錄之輸入/輸出函數曲線包含代表耳蝸非線性反應的5種曲線:單段型,平陡型,雙相型,飽和型,及非單一型,其中以飽和型,雙相型,及單段型較常見。但平均輸入/輸出函數曲線中只見到線性反應曲線,且斜率由0.50-0.78,大致隨頻率升高而變大。變頻耳聲傳射閾值與純音聽力閾值相關性以0.75,3,及4 kHz較高,若以平均閾值±2 SD為準,變頻耳聲傳射閾值預估正常純音聽力的準確率可達96.4%。

Distortion product otoacoustic emissions (DPOAEs) at the 2f1-f2 frequency were recorded from 26 normal hearing ears in response to equal-level primary tones. We recorded DP grams and inputoutput functional curves in 1/2 octave steps from 0.5-8 kHz at different primary tone levels. In addition, we checked the relationship between visible thresholds of DPOAEs and thresholds of pure tones. DPOAEs were obtained reliably from 25 ears (96.2%) with visible thresholds from 15-60 dB SPL. The average DP gram demonstrated a bilobed contour having a low-frequency plateau at 0.75-1.5 kHz and a high-frequency maximum at 6-8 kHz. Individual input-output function curves revealed 5 styles: single segmental, flatsteep, biphasic, saturating, and non-monotonic curves. Saturating, biphasic, flat-steep and non-monotonic styles may be the results of nonlinearity reaction in the cochlea. However, only linear curves with slopes from 0.50-0.78 were noted in the average input-output functional curves. The slope became steeper with increasing frequency of primary tones. In general, the correlations between thresholds of DPOAEs and pure tones were low. However, higher correlations were found in measurements of 0.75, 3 and 4 kHz. Also, we could reliably identify normal-hearing ears with the limit of DPOAE threshold mean ± 2 standard deviation. According to our results, DPOAE-recording is a potential tool for hearing screening.
