感音性聽障之變頻耳聲傳射

Distortion Product Otoacoustic Emissions of Sensorineural Hearing Loss

背景:為了作為日後臨床應用的基準,本研究測量分析感音性聽障耳之變頻耳聲傳射特性。 方法:在19側感音性聽障耳使用相同強度的兩刺激音,誘發記錄2f1-f2頻率之變頻耳聲傳射,以不同刺激音量求得自0.5至8kHz,每1/2八度音頻率的變頻耳聲傳射圖及輸入/輸出函數曲線,並探討變頻耳聲傳射閾值與純音聽力閾值的相關性。 結果:以70dB SPL刺樂音量在不同聽閾求得的平均變頻耳聲傳射強度約為-0.25至-11.3Db,如果變頻耳聲傳射的強度小於0dB,則聽閾就劣於30dBHL。輸入/輸出函數曲線分布,以線性的兩點型及低反應單段型為最多,若加上無反應者,即佔了全部記錄的90%以上。各單頻純音聽力閥值與變頻耳聲傳閾值間的相關係數(correlation coefficient)由0.09至0.807,其中以1.5及2 kHz間的相關係數最高。 結論:變頻耳聲傳射可用來作為輕、中度聽障耳外毛細胞受損後的殘餘耳蝸功能評估,也可作為聽力篩檢的工具之一。(中耳醫誌 1997;32:423-429)

Background: For observing the potential of distortion product otoacoustic emissions (DPOAEs) in clinical use, we study the characteristics of DPOAEs in ears of sensorineural hearing loss. Methods: Distortion product otoacoustic emissions at the 2f1-f2 frequency were recorded from 19 ears of sensorineural hearing loss. We recorded DPgrams and input-output functional curves in 1/2 octave steps from 0.5-8 kHz at different primary tone levls. In addition, we checked the relationship between visible thresholds of DPOAEs and thresholds of pure tones. Results: The average amplitudes of DPOAEs were -0.25 to -11.3 dB with 70 dB SPL stimuli. The recorded amplitude of DPOAE was less than 0 dB from ears with hearing thresholds higher than 30dB. Most input-output function curves were linear and low response (e.g. single-segment, two-point curvese and no response). The correlations between thresholds of DPOAEs and pure tones were low. However, higher correlations were found in measurements of 1.5 and kHz. Conclusions: according to our results, DPOAE-recording could be useful for estimating residual cochlear function in ears of mild and moderate sensorinerual hearing loss. It can also be one of effective tools for hearing screening.
