Periorbital Arteriovenous Malformation Case Report
眼眶周圍的動靜脈畸形－病例報告
摘要
動靜脈畸形的治療，尤其位於眼眶及其周圍，對外科醫生而言仍一大桃戰。近年來由於放射學技術的進步、血管攝影的正確定位及術前血管栓塞技術的進步，大大的提高了靜脈畸形外科手術的可行性，並降低手術的危險性。結合術前血管栓塞術及外科切除是目前對於動靜脈畸形較適合的治療方式。我們報告一位右眼眶上的動靜脈畸形的病例。術前將右淺顳動脈及右頜動脈栓塞，並於二十四小時內再行外科完全切除術，術後兩年追蹤，病人情況良好，沒有動脈畸形的復發。

This is a case of orbital arteriovenous malformation presenting with palsating mass, about 1×1×0.5cm^3 in size, with thrill over the right upper orbital region, and black discoloration. Selective angiography of right (Rt.) external carotid artery revealed an arteriovenous malformation at Rt. supraotbital region, fed chiefly by Rt. superficial temporal a. and Rt. internal maxillary a., immediate venous drainage from the malformation was directly to the external jugular vein. Preoperative intra-arterial embolization of right superficial temporal a. and internal maxillary a. with gelform pieces and glue was performed, followerd by excision of AVM within 24 hours. After two year's follow up, the post-operative course was uneventful, no recurrence of AVM was found.
