Increasing the Keratinized Gingiva of the Teeth with a Buccally Positioned Pedicle Flap from the Palatal Mucosa-Case Report

For decades, many mucogingival surgeries have been developed to increase the zone of keratinized gingiva. This article describes a modified rotated pedicle flap designed to increase the width of the keratinized gingiva. This procedure is suitable for patients with chronic periodontitis who have significant interdental spaces between their maxillary posterior teeth despite active periodontal therapy. Not only can this technique augment the amount of firm keratinized tissue to prevent mobile marginal tissue recession, but it can also move the tensile muscle fiber and frenum which may pull the gingival margin apically. The results of this surgery demonstrate that the band of keratinized gingiva from the pedicle flap significantly slows the recession of the marginal tissue. Furthermore, it increases the vestibular depth, thereby improving the stability of the removable denture and facilitating the positioning the I-bar of the denture. The color match of the grafted tissue is also natural and esthetically acceptable.
