Clinical Application of implant-supported Bilateral Distal Extension Removable Partial Denture-Case Report

Distal extension removable partial denture (RPD) bases present a number of design and maintenance challenges, as support is required from the teeth, the mucosa, and the underlying residual ridges. A 53-year-old female patient with chronic periodontitis, root caries, and incomplete root canal filling of multiple teeth came to the Department of Dentistry, China Medical University Hospital and asked for treatment for periodontal disease and full-mouth rehabilitation. Periodontal therapy and removal of several teeth with a poor prognosis resulted in a completely edentulous maxilla opposing a class Ⅰ (Kennedy classification) bilateral partially edentulous mandible. Two implants were placed in the first molar region of the mandible bilaterally, and finally an upper complete denture and a lower cast metal RPD with an attachment design were constructed. The patient was satisfied with the restoration, as she was able to perform routine oral functions and maintain good oral hygiene. The prosthetic design changed the Kennedy classification of the mandibular partially edentulism from class Ⅰ to class Ⅲ, and the bilateral distal single implant placement changed the lower RPD from a tooth/tissue-supported to a tooth/implant-supported condition. We concluded that dental implants with attachment designs incorporated into an RPD can improve denture retention, stability, and support.
