Autotransplantation, or autogenous tooth transplantation, is a feasible and economical treatment modality in replacing unrestorable, hopeless or missing tooth when a suitable donor tooth is available. This paper presents a successful case of bilateral autotransplantations from mandibular third molars to bilateral maxillary second molars. Five-year after treatment, the clinical and radiographic examinations of the transplanted teeth revealed satisfactory and stable outcome, with no pathologic change endodontically or periodontically. It is concluded that optimal selection of donor tooth and recipient site results in successful autotransplantation and may be considered as a treatment alternative to conventional or implant prosthetic rehabilitation.
