完整發育之左上顎第三大臼齒牙齒傳統自體移植合併之非手術性根管治療：病例報告

根據美國牙髓病學家協會（American Association of Endodontists, AAE）於2020年1月指引提到，根管治療科醫師在保留牙齒的治療選項之中，牙齒自體移植（autotransplantation of teeth, ATT）、牙根切除術（root amputations）、快速牙齒突出術（rapid extrusion）以及蓄意再植術（intentional replantation）都是可以幫助病患維持自然齒列的方式。牙齒自體移植（ATT）的目標，就是在同一位患者身上把具有功能性的牙齒移植到適當位置。近四十年研究從原本成功率約五成的術式，慢慢演進成更高的成功率，可能主要影響的因素有移植齒（donor tooth）的牙周狀況是否良好、移植齒的牙根型態及大小、移植齒的牙根發育狀況、口外乾燥時間長短、拔牙時候創傷程度等因素。本病例為31歲女性患者，於2017年04月26日至本院門診就診。主訴為診所矯正醫師建議拔除左下顎後牙，並以智齒取代其位置。經過臨床檢查及環口放射線學檢查評估後，合併口腔外科醫師及牙髓病科醫師協同治療，同時進行牙齒自體移植及根管緊急處理，後續約診完成非手術性根管治療，術後追蹤病人一年狀況良好。

As the guideline in American Association of Endodontists (AAE) in January, 2020, autotransplantation of teeth (ATT), root amputations, rapid extrusion and intentional replantation are the treatment options for preserving natural teeth dentition. The goal of ATT is replanting functional tooth to the adapt position. Nearly 40 years of studies, the success rate is about 50% before. But the key factors of rising success rate are the periodontal condition, root morphology, root development stage of donor tooth, the extraoral dry time and the damage from extraction. We present a case of maxillary third molar transplanting to mandibular second molar area. The 31-year-old patient came to our department on 2017/04/26. She complained about her orthodontic dentist suggested her to have tooth autotransplantation in our hospital. After several clinical exam and radiographic exam were done, we arranged the surgery appointment which combined OMS doctor oral surgeon and endodontic dentist for tooth autotransplantation and endo emergency treatment at same time. Furthermore, we finished the nonsurgical endodontic treatment and kept follow up a year in good condition.
