摘要
纖維肉瘤是一種在頭頸部罕見與獨特的惡性腫瘤，它是起源於間質纖維母細胞型組織，而且有多樣異質性的特徵。纖維肉瘤表現出一種未分化纖維母細胞增生，並且形成膠原與網狀纖維。它可以發生在頭頸部任何區域，但很少發生在顎骨區，特別是上顎骨更少。我們報告一位51歲女性患有上顎纖維肉瘤，在其右上顎齒槽骨區有中等程度紅腫且表面呈現中央壞死的腫瘤。在手術前化學治療之後，腫瘤明顯縮小，病人就拒絕進一步廣泛性切除手術，而只願意接受化學治療。然而在5次化學治療之後，腫瘤繼續增大且侵犯到頰部空間、咀嚼空間、副咽空間、顳下窩、眼球後區空間、副鼻竇區空間。由於此巨大的腫瘤推擠導致眼球突出，並且也受到侵犯顱底，因此腫瘤已經無法切除。而病人也於最初診斷後26個月死亡。

Fibrosarcoma is a rare and unique malignancy found in the head and neck region. It is mesenchymal and fibroblastic origin with diversely heterogeneous characteristics. Fibrosarcoma reveals anaplastic fibroblastic proliferation and the formation of collagen and reticulin fiber. It can occur at any location of the head and neck area, with very rare in the jaw especially in maxilla). We report a 51-year-old female with maxillary fibrosarcoma. This moderate reddish tumor with a central necrosis surface occupied the right maxillary alveolus. It was obvious shrinkage after preoperative chemotherapy. However, patient declined further radical surgery and received chemotherapy only. After 5 courses chemotherapy, tumor still increased and extended into buccal space, masseterice space, parapharyngeal space, infratemporal fossa, retrobulbar space, and parasinus space. The huge tumor caused right eye global protrusion and skull base invasion. Unfortunately, it became unoperable and the patient died 26 months after initial diagnosis.
