Odontogenic keratocysts are common cysts of jaws that are locally aggressive and have a high recurrence rate. They are often associated with impacted teeth because they commonly occur in the posterior molar region of the lower jaws. As the tumor grows larger, these impacted teeth may be pushed into various areas of the jaw, making treatment difficult. Our case report is an odontogenic keratocyst involving the posterior molar region of the left mandible with a third molar displaced to the border of the mandible with a complex root pattern. The cyst was enucleated and peripheral osteotomy was performed with simultaneous extraction of the ectopic third molar. The follow-up after 6 months showed no sign of recurrence with good radiologic ossification. The management of the odontogenic keratocyst and the ectopic impaction was also discussed.
