Odontogenic Keratocyst over Buccal Aspect of Mandibular Canine Area-A Case Report and Literature Review

下顎犬齒區齒源性角化囊腫-病例報告與文獻回顧

摘要


Odontogenic keratocyst (OKC) was reclassified as an odontogenic cyst back in 2017 from kertocystic odontogenic tumor, which accounts 3-11 % of all cysts in the maxillofacial region. The most common occurrence site is the posterior body and ascending ramus of mandible. Clinical signs of OKC include pain, swelling, discharge or a result of secondary infection. In radiography, OKC usually demonstrates a well-defined radiolucent area with smooth and corticated margin. Both the clinical and radiographic findings of OKC are not diagnostic. The diagnmosis of OKC is based on the histopathological examinations. There are still many debates of treatment modalities because of its varying recurrence rates reported. We reported a mandibular OKC over a rare location-canine region of a 26- year-old female. Conservative surgical intervention with Carnoy's solution soaking was performed under general anesthesia. In addition, we reviewed the literature focused on different treatment protocols of OKC.

齒源性角化囊腫約佔上下顎骨囊腫的3%〜11%,最常發生在下顎後牙區或是下顎上升枝區。臨床上的症狀包括疼痛、腫脹、分泌物流出或次發性感染,影像上則是常可見一邊緣清晰之放射線透射性病灶,診斷上很難與其他齒源性囊腫或良性腫瘤做鑑別診斷,還是需要組織病理學檢查才有辦法診斷。治療上因其容易復發的特性,目前仍存在許多爭議,本文提出一發生於罕見位置-下顎犬齒區之齒源性角化囊腫,並回顧與治療計畫訂定相關之文獻做討論。
