Non-sebaceous Lymphadenoma of the Parotid Gland-A Case Report

Non-sebaceous lymphadenoma (NSL) of the major salivary gland is a rare benign tumor of unknown etiology, with only 52 cases reported in the English language literature. Here we report a case of a 64-year-old woman with a firm mass over the right parotid region. Magnetic resonance imaging demonstrated two enhancing nodules in the right parotid gland. One is in the posterior portion and the other is in the inferior portion of the right parotid gland. Superficial parotidectomy with preservation of the facial nerve was performed. Pathology reported as a benign non-sebaceous lymphadenoma and a benign reactive lymph node hyperplasia within the normal salivary gland tissue. According to the literature review, the prognosis of NSL is good, except in one case where a malignant transformation was reported. More studies are needed to avoid misdiagnosis and to better understand NSL.

非皮脂腺淋巴腺瘤是一種罕見的良性腫瘤，臨床通常為無痛且緩慢生長之腫塊，好發於腮腺，回顧過去的文獻僅有52例個案被報導。在組織病理學上為淋巴腺瘤內含大量的淋巴組織浸潤及多樣態的上皮組織增生，且同時表現無皮脂腺分化。有別於其他腮腺腫瘤，非皮脂腺淋巴腺瘤缺少特殊的臨床表徵及組織學特徵，進而導致術前及術後診斷的困難，須逐一排除其他腮腺腫瘤後才能確立診斷。本文提出之病例為一位64歲女性，右側腮腺區域出現緩慢生長的實質腫塊。經核磁共振影像學檢查後發現於右側腮腺中有兩個結節的顯影，一個位於腮腺淺葉的後部，另一個位於腮腺淺葉的尾部。手術方式選擇腮腺淺葉切除手術並保留顏面神經。組織病理報告顯示為正常唾液腺組織內含非皮脂腺淋巴瘤及反應性增生淋巴結。病人術後復原良好，經過半年追蹤無明顯復發或轉移。根據文獻回顧，只有一個非皮脂腺淋巴腺瘤案例出現惡性轉變，其餘手術切除後皆預後良好，並無復發或是轉移的情形被報導。本文中針對非皮脂腺淋巴腺瘤的診斷、治療及預後進行文獻回顧及討論。
