矯正上頷犬齒與第一小臼齒牙齒錯位之空間重新分配及贋復重建－病例報告

摘要

牙齒錯位意指在同一象限中任兩顆或以上之恆牙發生萌發位置互換的情形。至今對造成牙齒錯位的原因無一定論，但可推得與基因或乳牙滯留或外傷有關。上頷犬齒牙齒最易與第一小臼齒錯位，偶爾與側門牙互換。而牙齒錯位對牙科醫師而言更是一大挑戰。如何兼顧美觀與功能，需要詳細的評估與完整的治療計畫。本病例為一左上乳犬齒滯留，左上側門齒先天性缺牙，右上側門齒為錐狀齒，及左上犬齒與第一小臼齒的牙齒錯位。在歷經一年半年的矯正治療時間後與家庭牙醫學科合作，將口內的錐狀齒與先天性缺牙做復重建。本篇還探討牙齒錯位的發生率、治療方法及其選擇，以及治療過程中會遭遇的問題與注意事項。

Tooth transposition is a dental anomaly characterized by the exchange in position of 2 or more teeth within a quadrant of the dental arch. Although the mechanism of tooth transposition is not clear, it seems to be closely related to genes or retained primary tooth or trauma. Maxillary permanent canine usually transposes with the first premolar and occasionally with lateral incisor. Tooth transposition is a big challenge for many dentists. The present case report describes a situation where the left canine to the left first premolar complete transposition was present along with a congenital missing left lateral incisor and the peg-shaped right lateral incisor. After a period of one and half year of the orthodontic treatment, we had performed prosthodontic treatment. The article will discuss the prevalence, clinical diagnosis and the orthodontic, prosthodontic treatment.
