自體上皮下結締組織移植作為在下顎大臼齒二級根尖分叉侵犯的阻隔膜治療

The Use of Autogenous Subepithelial Connective Tissue Graft as a Barrier for the Treatment of Class II Furcation Involvement in Lower Molar

摘要


患有牙根分叉侵犯的牙齒是牙周治療較困難的地方,因為這類病灶清潔不易且對翻瓣手術的治療效果較差。目前針對下顎二級根尖分叉侵犯的治療認為是引導組織再生術(GTR)可達到最可預期的再生效果。然而近期內也有許多研究報告認為使用自體上皮下結締組織移植作為牙周再生的阻隔膜也可達到與引導組織再生術類似的附連增加及牙根覆蓋的效果。本文提出一位五十歲患有局部慢性牙周炎的女性,其兩側患有二級根尖分叉侵犯的下顎大臼齒,分別經過引導組織再生術及上皮下結締組織移植阻隔膜治療後,皆可得到類似的再生效果。因此認為在條件良好下,上皮下結締組織移植可供作為類似病例的治療選擇。

The class II furcation involvement of teeth are more difficult for periodontal treatment; as such defects are not easy for cleaning and not effective with open flap surgery. So far, the most ideal treatment of lower molars class II furcation involvement has been considered to guided tissue regeneration (GTR), which can have the most predictive regeneration effect. However, in recent years, there have also been many research reports indicating that using autogenous subepithelial connective tissue graft (SCTG) as barriers can also have the similar regeneration and root coverage effect to GTR. In this case, a fifty-year-old woman suffered from localized chronic periodontitis, whose bilateral lower first molars were class II furcation involved. After GTR and SCTG treated individually, similar regenerative effects can be found. As a result, under good circumstances, SCTG can be treatment options for similar cases.
