摘要

牙周組織和牙髓組織在解剖構造上可以副根管、側根管、根尖開口和牙本質小管互相交通，所以當有一方出現病灶時，便有可能藉由上述的通道影響到另一方。當牙齒出現牙周合併牙髓問題時，原則上須先進行根管治療，之後再視情況進行牙周治療。本文提出一慢性牙周炎患者的整體治療病例，其中47區域經由根管治療後，再經由引導組織再生手術合併骨移植來治療根叉侵犯以及骨內缺損。接著針對治療區域進行臨床牙周探測數值與放射影像分析，來觀察長期追蹤下再生牙周組織的效果。

Periodontal tissue and pulp tissue can communicate each other by some anatomical structures, like accessory canal, lateral canal, apical foramen and dentinal tubule. Once lesion appears, it is possible to influence by the above passage to the other side. The treatment of peri-endo combined lesion needs endodontic root canal treatment first, then comes periodontal treatment or not. This paper presents the overall treatment of a patient with chronic periodontitis, where 47 received root canal therapy first, then using guiding tissue regeneration with bone graft was used to treat furcation involvement and bony defects. Followed by clinical data and radiographic analysis of the treatment area to observe the long-term effect by guiding tissue regeneration.
