Rehabilitation of upper anterior region is a challenging mission due to the influence of esthetics and social activity. Therefore, The patient's treatment expectation of this area could not be on a par with any other intraoral area. Digital smile design is a concept of analyzing the oral condition and simulating the result. It can visualize the smile design via software, to facilitate a discussion between dentists and patients before establishing final treatment decision. With the application of laser in periodontal plastic surgery, the stability of soft tissue margin and esthetic outcome are more predictable. In this report, we present a case reconstruction upper anterior gingival level and discolored teeth with all-ceramic crown and porcelain veneer through digital smile design and Er: YAG laser assisted crown lengthening surgery. The result remains stable in follow-up.
