Comprehensive Periodontal and Implant Treatment in a Smoking Patient: A Case Report

摘要

本案例討論整體牙周病與植牙治療計畫於一49歲男性，診斷為廣泛型中度局部型重度慢性牙周炎，且為抽菸患者。治療前臨床檢查可見多處牙菌斑堆積、軟組織發炎與附連喪失；放射線學檢查結果有廣泛性水平骨缺損與局部性骨缺損於上顎臼齒區域。經牙周基本治療與戒菸之後，後續進行牙周手術、牙周再生手術與植牙重建。術後一年追蹤，牙周狀況改善顯著顯著且穩定，牙菌斑與出血指數分別降為18％與11％。此外，探測深度98%為1-3毫米，97%的位置有附連獲得。臼齒手術區域有放射線骨密度增加，植牙處也無放射線周圍骨喪失。定期牙周支持性治療且整體預後良好。

This case report discusses comprehensive periodontal and implant treatments in a 49 y/o smoking male with generalized moderate and localized severe chronic periodontitis. Baseline clinical examination revealed generalized plaque deposition, gingival inflammation and loss of attachments. Radiographic examination showed generalized horizontal bony defects and localized angular bone defect on upper left molars. After non-surgical treatment and quit smoking, he received subsequent periodontal surgeries and regenerative therapy and implant therapy. One year follow-up after treatment, the improvement of periodontal condition was observed and remained stable. The plaque and bleeding index reduced to 18% and 11%. In addition, 98% sites were detected with 1-3mm probing depth, and 97% sites had attachment gain. Radiographic bone gain were noted at molars surgical sites, and no radiographic bone loss around implant. The patient received regularly supportive periodontal treatment and the overall prognosis was good.
