Using Non-microsurgical Method for Auricular Amputation Reconstruction - the Modified Baudet Technique

使用非顯微手術的方法對於耳廓截肢之重建-修改後之Baudet術

摘要


背景:耳廓的獨特解剖結構使外傷性截肢後耳廓重建相當困難,而且容易失敗,尤其是在人咬傷的病例。材料及方法:一名54歲的女性是人類咬傷的右耳外傷性截肢受害者。考慮到外傷性截肢的位置和受傷的機制,顯微再植手術的失敗率很高。為能挽救截斷之耳軟骨和保留耳部外觀輪廓,而回顧了關於耳廓重建之非顯微外科手術法的相關文獻。結果:使用修改後之Baudet氏法之階段性手術。大部分的耳軟骨都被挽救且保留,結果得到令人滿意的耳部輪廓而且沒有任何感染以及傷口癒合不良之併發症。結論:耳廓的顯微手術再植十分困難並且容易失敗。修改後的Baudet氏術為耳廓截肢重建提供另一種簡單並且可行的選擇。

Background: The unique anatomic structure of the pinna makes auricular reconstruction difficult and prone to failure after a traumatic amputation, especially in cases of human bite injury. Materials and Methods: A 54 years old female was a victim of right ear traumatic amputation by human bite. Considering the amputation level and mechanism of injury, high failure rate of microsurgical replantation was anticipated. Literatures of auricular reconstruction by non-microsurgical technique were reviewed for the amputated cartilage salvage and ear contour restoration. Results: Staged surgery with the modified Baudet technique was performed. The amputated ear cartilage was mostly salvaged and satisfactory ear contour was restored without infection or wound dehiscence. Conclusion: Microsurgical replantation of pinna is difficult and has low success rate. The modified Baudet technique provides an alternative simple and feasible choice of reconstruction for auricular amputation.
