照顧一位顏面燒傷婦女之護理經驗
A Nursing Experience of Caring for a Patient with Facial Burn
摘要
本文描述協助一位顏面燒傷婦女住院照護經驗。於2011/03/22~2011/05/03護理期間，藉由觀察、會談收集資料、並運用Gordon十一項健康功能模式評估分析，發現個案有疼痛／與傷口換藥及復健有關、自我照顧能力缺失／與燒傷疤痕增生及關節活動限制有關、身體心像改變／燒傷傷口致外觀改變等護理問題。過程中以主動關懷、支持、陪伴、傾聽及鼓勵等技巧，提供所需相關知識及照護，幫助個案在身、心、社會等方面的適應，且接受身體心像改變的事實而順利返家。期望此護理經驗有助提升護理人員對燒傷病人面臨傷口導致疼痛、身體心像改變問題產生時，能有計畫適當介入護理措施，以提升護理照護及病人的生活品質。

This article describes the nursing experience of caring for a patient with facial bum. Data were collected through observation and interview during the nursing period from March 22 to May 3, 2011. After an assessment using Gordon's 11 Functional Health Pattern Assessment Tool, the identified nursing problems were pain from wound dressing change and physical rehabilitation, self-care deficiency because of hypertrophic scars and limited joint mobility, and body image changes associated with burned facial wounds. During the nursing period, the author provided the patient with wound care-relevant knowledge and care using the following skills: active caring, support, companionship, and listening. The patient was able to be discharged smoothly with the assistance of coping in physical, psychological, and social aspects. We hope that this nursing experience can be used as a reference for other nurses who care for patients with bum wound pain and body image issues.
