Total talar dislocation is a rare injury;the open type is usually more frequent. Immediate open reduction can decrease late complications. Misdiagnosis may occur as an ankle dislocation. This case is a 44-year-old male who suffered a traffic accident. No open wound was noted, and the malleolar bones were intact. Closed reduction failed, so emergent open reduction and internal fixation were done. Total talar fracture dislocation was noted, and the talar head was entrapped between the extensor hallucis longus and extensor digitorum longus tendons. Early diagnosis and prompt treatment can produce a good prognosis.
