本研究目的為在上下階梯測試中,探討5種肌內效貼紮狀態(髕骨矯正貼紮、髖外展貼紮、支持足弓貼紮、合併貼紮與無貼紮)對髕股骨疼痛受試者肌電特性與減痛效益的影響。利用表面肌電圖收集系統,收集13位髕股骨疼痛患者進行上下階梯動作,分析貼紮對股內斜肌與股外側肌的肌電比值與收縮反應時間差的影響,並用視覺疼痛量表評估貼紮的效益。研究結果發現:不同的肌內效貼紮狀態在上下階梯時的股內斜肌/股外側肌肌電比值與視覺疼痛量表指數均無顯著差異。和無貼紮相比較下,使用肌內效貼紮可降低股內斜肌收縮反應時間延緩現象,特別在上階梯時,髕骨矯正貼紮和髖外展貼紮可促使股內斜肌出現提早收縮現象;而下階梯時的支持足弓貼紮也會出現股內斜肌提早收縮現象。本研究結論:經由適當的肌內效貼紮方式,在膝關節伸直運動時,促使股內斜肌和股外側肌達到內外側肌肉收縮的時序平衡,以穩定髕股骨關節,減少髕股骨疼痛的發生機率。
The purpose of this study was to investigate the effect of five kinesio taping conditions (hip abductor taping, patellar correction taping, arch support taping, combined taping and no taping) on the electromyographic (EMG) characteristics and pain release in subjects with patellofemoral pain syndrome. Thirteen subjects with patellofemoral pain syndrome were participated in this study. Surface EMG system was used, and two electrodes were placed on the vastus medialis oblique and vastus lateralis of subjects. The ration of vastus medialis oblique to vastus lateralis EMG ratio, the onset time difference during step ascent and descent, and the visual analogue scale were analyzed. No significant difference was found in the EMG ratio and the visual analogue scale among different taping conditions. However, four kinesio taping conditions could significantly diminish the onset delay of vastus medialis oblique compared to no taping condition. Specifically, hip abductor taping and patellar correction taping could elicit an early onset of the vastus medialis oblique during step ascent. Arch support taping could elicit an early onset of the vastus medialis oblique during step descent. These findings conclude that the use of kinesio taping could help the medial-lateral muscle onset balance in quadriceps during knee extension. It might be of assistance in maintaining the patellofemoral joint stability and reducing the risk of patellofemoral pain syndrome in subjects.
