某公立醫院附設產後護理機構探討產婦之純母乳哺餵經驗

Understanding Maternal Experiences with Exclusive Breastfeeding

摘要


本機構於九十八年一月至十二月期間,以臺大醫院精神科林朝誠醫師之IDS-SR量表評估149位產後3-7天的婦女,分析結果發現有10%婦女有輕度憂鬱,再進一步瞭解其原因之所在,其中75%婦女表示在母乳哺餵過程有困難所困擾,故運用質性研究方式瞭解純母乳哺餵婦女其核心問題之所在。經立意取樣,收集9位婦女分別在入住第3天及14天以兩個問題,問題一:當餵寶寶時有甚麼感覺?。問題二:你覺得餵母奶最需要那些支持?進行面對面深度訪談,其獲得12篇逐字文稿,經由歸納分析呈現三個主題,主題一、母乳哺餵成敗都在母親身上,太沉重。主題二、為母則強,母性角色任務的傳統制式思想。主題三、支持系統的不活絡及效能不足。在高度變動性社會環境,凡事以效率、有效、企業管理的生活進行,但母乳哺餵顯示萬物的自然及不減的特性。藉由此研結果期望相關醫療機構及醫事人員能對產後婦女的哺餵母乳,提供有效、自然、簡單、愉悅的相關協助及護理措施。讓婦女能享受母乳哺餵的女性獨特任務,且是一段美麗的生命歷程。

From January to December, 2009, the IDSSR scale developed by Dr. Chao-Cheng Lin at Department of Psychiatry of National Taiwan University Hospital was used to assess 149 women who were 3-7 days postpartum at this nursing institution. The analysis found that 10% of the mothers experienced mild depression. A further investigation of the causes of this depression found that 75% of the women were experiencing mild depression because they had encountered problems during breastfeeding. Qualitative research was applied to identify the core problems encountered by the women who chose to exclusively breastfeeding and these individuals were identified for this part of the study by purpose sampling. Two questions were presented to these nine women on the third and fourteenth day postpartum and their answers were recorded. Question 1 was ”How do you feel during breastfeeding?” and Question 2 was ”What supports are most needed during your breastfeeding experience?”. Face-to-face in-depth interviews were conducted, and twelve verbatim transcriptions were obtained. Using inductive analysis, three topics were identified. Topic 1: It is a heavy burden to a woman if the success of breastfeeding must rely only on the individual mother herself. Topic 2: Being a mother means being strong-this being the traditional idea about the roles and tasks of a mother. Topic 3: A lack of support systems and significant inefficiency of the support systems that are in place. In a rapidly changing social environment, everything seems to need to be conducted efficiently and effectively like a business. However, breastfeeding involves the natural and eternal characteristics of all things that are present in the world. It is hoped that the results obtained in this study will help relevant medical institutions and personnel to provide postpartum women with natural, simple, pleasurable assistance and nursing measures that will enable them to enjoy this unique task of women and allow them a beautiful journey through life.
