The metastasis rate of cutaneous squamous cell carcinoma of the head and neck is as rare as 5%. The commonest metastatic areas are parotid gland and neck lymph nodes. We report an 80-year-old man with a right side preauricular and right neck masses. He was a victim of squamous cell carcinoma of right face and underwent wide excision 18 months ago. However mass lesions developed over right preauricular area and lower neck about 6 months ago. After superficial parotidectomy and radical neck dissection, the pathology revealed moderately -differentiated metastatic squamous cell carcinoma. Cutaneous metastatic squamous cell carcinoma involving the parotid gland and neck is an aggressive disease. Cumulative disease-specific survival at 5 years is 61%. More aggressive surgery such as total parotidectomy, radical neck dissection and adjuvant radiotherapy are recommended.
