上頜竇之血管內乳突狀內皮增生
Intravascular Papillary Endothelial Hyperplasia of the Maxillary Sinus
摘要
鼻竇之血管內乳突狀內皮增生(intravascular papillary endothelial hyperplasia, IPEH)是一種相當罕見的良性病例，回顧文獻記載少有提及。本科於2001年6月經歷一名65歲男性病患，主訴過去2個月持續膿鼻涕、鼻塞，偶有流鼻血現象，經電腦斷層檢查後發現右側上頜竇有不透光區。於是施予鼻竇內視鏡手術，病理報告為血管內乳突狀內皮增生。血管內乳突狀內皮增生在組織學上特徵與血管肉瘤(angiosarcoma)相似，必須加以鑑別診斷。本例門診追蹤迄今無任何復發跡象。

Intravascular papillary endothelial hyperplasia (IPEH) is a benign endothelial proliferation. Masson first described it in 1923. It is rather a reactive lesion than neoplasm. IPEH often occurs in the head and neck and extremities, but rarely within the paranasal sinus. According to the English literature, there are only three previous reports of IPEH within the paranasal sinus. We present a 65-year-old male patient with IPEH of the right maxillary sinus who suffered from rhinorrhea and bloody discharge for two months. We removed the lesion by endoscopic sinus surgery. So far, no local recurrence is noted. IPEH is similar to angiosarcoma in histological feature. Although the incidence is rare, IPEH must be considered in the category of intranasal tumor and differentiated from the angiosarcoma.
