摘要

本文探討一位16歲青少女，因罹患陰道隔膜接受陰道重建術之護理經驗，於民國91年8月28日至9月17日護理期間，以Gordon十一項健康護理評估，確認個案有舒適情況政變、身體心像紊亂、知識缺失、照顧者角色緊張等護理問題，經由護理措施的介入，運用接受性態度、傾聽、資訊提供，陪伴渡過手術及康復歷程，促進身心舒適及維護身體心像完整性，並協助解決疾病帶給家庭的壓力及問題，讓整個護理過程除了照顧病患外，也同時照顧到家屬的需要。

This article reported the nursing experience of a 16 years old teenage girl who received vaginal reconstruction due to transverse vaginal septum. Using Gordon's functional health assessment guide, The nursing staff assessed this patient and found the following problems: pain, body image disturbance, knowledge deficit, and caregiver role strain, etc. In the nursing process, the author tried to encourage the patient expressing her thinking, help her lace the change of body image. Nursing care is also required to help the family alleviate stress and tension. The nursing procedures not only care for patient but also help the entire family.
