運用行為治療於一位亞斯伯格症個案之護理經驗

The Nursing Experience of Applying Behaviour Therapy as Treatment to a Subject with Asperger's Sydrome

摘要


本文乃運用行為治療協助一位亞斯伯格症個案,透過與家屬、學校老師會談深入了解個案在家中及學校的行為,並實際觀察個案臨床行為表現,收集相關資料,發現個案之健康問題有一、潛在性暴力-針對他人/與暴力攻擊他人有關;二、社交互動障礙/與缺乏社交技巧有關,筆者與個案、家屬、醫療團隊共同討論後,依個案特性擬訂定行為矯正計劃,透過角色扮演方式,作角色模範,讓個案學習;於三個月的治療期間(95.6.6~9.12),矯正因疾病所導致的暴力、擅取物品與社交技巧障礙等問題,個案的暴力攻擊他人的行為由平均2次/1天降為平均2次/1個月,其不當行為明顯獲得改善,並可適時運用正向社交技巧與人互動,開拓更佳人際關係,順利於開學後二週返校上課,藉此顯示行為治療對亞斯伯格疾患不當行為的矯正有顯著的成效。

This article is based on applying behaviour therapy to treat a subject with Asperger's Sydrome. From the behaviour information obtained through in-depth conversations with the subject's family and school teacher and actual observation of the subject's clinical behaviour, it was determined that the subject's main health issues were related to the underlying aggressive behaviour towards others, as well as causing physical harm to others, and the lack of social interaction and social skills. A behaviour modification program, based on role playing and role modelling, was established after collaboration between the authors, the subject, the subject's family, and the medical team. Subjects of inappropriate behaviour significantly improved after 3 months of treatment to rectify violent behavior caused by the illness, such as issues of arrogating items and the lack of social skills, and the subject’s violent episodes towards others decreased from an average of twice per day to twice a month. In addition, the subject was able to use appropriate social skills to interact with others to establish positive relationships and successfully return to the classroom. This demonstrated that the application of behaviour therapy has a significant effect in rectifying inappropriate behaviours caused by Asperger's Syndrome.
