氣功對巴金森氏症患者運動與非運動症狀之療效:系統性文獻回顧暨統合分析

Effects of Qigong on Motor and Non-motor Symptoms of Parkinson's: A Systematic Review and Meta-analysis

摘要


背景:巴金森氏症(Parkinson's Disease, PD)是慢性中樞神經系統退化性疾病,服藥雖是主要的治療方式,卻無法停止疾病及運動障礙的持續惡化。目的:探討氣功介入對巴金森氏症患者的運動及非運動症狀之療效。方法:以系統性文獻回顧分析氣功於巴金森氏症患者運動與非運動症狀管理之運用,並使用統合分析檢驗氣功的實際介入成效。結果:於4個英文及2個中文資料庫搜索共得21340筆文獻,篩選後納入5篇文獻檢試氣功介入實驗之成效。氣功能改善運動症狀,但對非運動症狀則多數缺乏顯著效益。結論:氣功可改善PD患者如動作能力、起立走計時、單腳閉目站立測試、肌肉硬度測試、手眼協調功能等運動功能症狀之測試結果,可增加平衡能力,降低跌倒風險。建議未來的研究針對不同PD病程探討氣功之效益,以建立更適合PD病患的氣功照護指引。

Background: Parkinson's disease (PD) is a chronic degenerative disease of the central nervous system. Though medication is the main treatment, the disease and its related motor dysfunctions continue to progress. Method: To examine the effect of Qigong on motor and non-motor functions of Parkinson's. Result: 21,340 citations were retrieved from four English and two Chinese databases. Five research articles were included for this study. The practice of Qigong is effective for promoting motor functions in parkinson's, but not for the non-motor functions. Discussion: The exclusion criteria that excluded Tachi interventions that are usually confused with Qigong allowed only five studies selected for this study to present the true effect of Qigong. Besides UPDRS III, included studies that measured varied variables and made data of each variable were provided by only one study. This is the limitation of this study. It also presents insufficient confidence to the evidence of Qigong studies and further well-designed studies are encouraged. The lack of support to the effect of Qigong on non-motor functions of PD might also be introduced by insufficient time of Qigong engagement. As for the degeneration progress, Qigong might not be helpful for PD patients because of them being unable to practice this exercise. Therefore, when and how Qigong training is delivered to PD patients becomes of critical concern. Conclusions: Qigong may improve motor functions in Parkinson's. The improvement in balance functions may decrease the risk of falls. We suggest the future studies to develop a clinical practice guideline for Qigong by validating the functions of Qigong for every level of PD.
