本專案的目的是為提升門診癌症病人對化學治療認知的正確率,以促進其居家自我照顧的能力,並提升對護理指導之滿意度。臨床發現門診癌症病人對化學治療後返家所產生的副作用感到困擾,甚至有產生放棄治療的念頭,因而引發本專案探討的動機。經調查發現門診癌症病人對化學治療的正確認知率僅37%。分析其原因,在護理人員方面則為護理指導時間較短及片段式的衛教;在病人方面則有因年紀大、不識字、只聽得懂台語等問題;在工具方面則出現衛教小卡字體太小及種類不足等。本專案的解決之道在為等候治療的癌症病人播放國語或台語影音多媒體護理指導光碟、提供居家台語版護理指導教學錄音帶及修訂製作衛教圖卡本。實施改善方法後發現門診癌症病人對化學治療正確認知率提升至87%;對護理指導滿意度達4.5分;副作用困擾減輕;對癌症病人居家自我照顧能力確有其助益。
The purpose of this project was to improve the recognition accuracy rate of cancer outpatient's chemotherapy, and to advance the ability of home self-care, also to elevate the satisfaction of the nursing instruction. Clinical findings show that cancer outpatients are disturbed by the side effects of Chemotherapy at home. They even have the urge to give up the treatment. Thus it triggered the motivation of this project. The survey found that the proper recognition rate of chemotherapy treatment is only 37% among the cancer outpatients. Analysis of its causes: In the health care personnel: the time for nursing instruction was too short, and the health education was fragmented based; in the patients: due to older, illiterate and only understand issues in Taiwanese; the teaching tool: the font and size of the educational cards are small and inadequately prepared. The solution for these problems is: Play Mandarin or Taiwanese multimedia patient education CD-ROM for the cancer patients in hospital waiting room; Provide the Taiwanese version of nursing instruction tape for home use and revise health education picture handbook. After the implementation of the improvement steps, the proper recognition rate of outpatient's chemotherapy was up to 86%; the satisfaction rate of the nursing instruction was up to 4.5; disturbance by the side effects reduced; cancer patients did benefit from the home self-care.
