一位罹患巨細胞病毒感染外籍孕婦之急診護理經驗

A Nursing Experience of a Pregnant Woman with Cytomegalovirus Infection at Emergency Room

摘要


本文描述一位懷孕10週罹患巨細胞病毒感染的日籍經產婦,因發燒、全身起紅疹,在急診室因語言的隔閡加上擔心腹中胎兒受牽連,而引發一連串焦慮、不安情緒反應的護理經驗。筆者於2008年4月17日至2008年4月19日護理期間,以直接護理、觀察、會談與請案夫協助翻譯等方式收集資料,運用關懷理論以生理、心理、社會及靈性四大層面進行整體性評估,發現個案有營養狀況改變:少於身體所需要、舒適障礙、焦慮等健康問題。運用Watson理論的人性關懷本質,以親切的問候、主動的打招呼,讓其感受到關懷來建立信任關係;運用圖片來提供疾病相關的資訊與照護指引,減輕焦慮與不安並增加其認知,慢慢地由原先的忐忑不安到情緒平緩。希望藉此經驗分享照護不同國籍的經產婦在急診護理的獨特性。

The article is about the nursing experience of a Japanese pregnant woman who suffered from cytomegalovirus infection at 10-week pregnancy. Fever and skin rash were the main manifestations. Due to the language barrier and concerns of her fetus being affected, the woman expressed a series of anxiety reactions at the emergency room. During the nursing period of April 17 to April 19 in 2008, the author collected information by four approaches, including direct nursing care, observation, interviews and translation aid from the woman's husband. After the holistic assessment with caring theory in four major dimensions (physical, psychological, social and spiritual), the author found that she had the following problems, including nutritional imbalance, nutritional deficiency, discomfort and anxiety. Based on the essence of human caring in Watson theory, the trust relationship between the patient and the author was built through kindness and active greetings. With the use of pictures to provide illness-related information and nursing guide, the anxiety and trepidation were reduced and the cognition was enhanced, and her discomfort and tension were gradually reduced to calmness. Through this experience, the author wished to share this unique experience of nursing care of a foreign multipara at emergency room.
