社會人口結構的改變促使健康照護模式的轉變,整合性跨領域團隊照護及社區化連結持續性照護資源是現階段各領域積極努力的方向,政府不斷推動健康促進、疾病預防及慢性病照護管理相關的專案計畫,以提升民眾健康,改善照護成果,其中個案管理師是評估民眾健康照護需求、協調醫療團隊照護計畫、整合社區和照護資源、及追蹤成效的靈魂人物。在台灣個案管理照護模式已存在於各領域及醫療機構中超過二十年以上,但其執業能力尚未被明確定義及規範;遑論個案管理師訓練及資格認證需要建立具共識性的標準,導致角色功能發揮有限,照護成效進展緩慢,亟需整合各專科領域個案管理職務內涵,訂定該角色之執業能力標準。執業能力的好壞直接反應在照護品質與效能,而對執業角色能力的要求與提升,更能建立社會認同其價值與地位。本文探究目前台灣個案管理執業能力要求和訓練現況,比較國外實務後提出反思與建議,期待引發產官學界的關注,共同為建立優質個案管理照護制度的基礎而努力。
The changes in socio-demographic structure prompted the transition in healthcare model. Both integrated multidisciplinary team-based and community-based healthcare networking with resources for continuous care are the directions of vital efforts in various fields at this stage. In order to improve public health and care requirement, the government continues to promote project plans related to health promotion, disease prevention, and care management for chronic illness. Among them, case managers are soul figures who assess people's health care requirement, coordinate care plans with the medical team, integrate community and care resources, as well as tracking results. In Taiwan, the case management care model existed in various healthcare fields and medical institutions for more than two decades. So far, the competencies in regards to practice as a case manager have not been clearly defined and regulated, let alone setting standards for case manager training programs, neither consensus for qualification requirements and certification regulations. The current status has resulted in limited role functions of case managers and limited progress in care effectiveness. There is an urgent need to integrate the connotation of case management in each specialty areas and establish the standard of practice competencies for the role. The quality of practice competencies will directly reflected the quality and effectiveness of health care. The requirement and promotion of the ability to practice the role can surpass establishment of the societal recognition and hence its value and status. This article explores the current practice requirements and training programs of case management in Taiwan, hence comparing with foreign practice for reflection and recommendation. We look forward to initiate the awareness from the healthcare industry, government, and academia, for the purpose of working together to establish the basis of a high-quality case management care system.
