The Outcomes of Home Pharmaceutical Care on High Users of Medical Resources in Taiwan

摘要

臺灣藥師法於2007年修訂版上，第十五條藥師業務增加了第八款的藥事照護相關業務，這開啟了藥師依法執行藥事照護的業務。中華民國藥師公會全國聯合會（以下簡稱全聯會）連理事長於2009年6月帶領著全聯會幹部，向政府長官及立法委員溝通，並成功說服健保局於2010年撥出924萬總額預算，開啟一個試辦計畫：「全民健保高診次民眾就醫行為改善方案－藥事居家照護」，讓健保局臺北分局與高屏分局的八縣市先行試辦。這一年有42位藥師照顧808位個案，健保局給付藥師1 元，藥師所照顧個案在門診醫療費用上可下降3.3元。因此，2011年繼續此試辦方案，並擴大到全臺灣22縣市執行藥事居家照護。第二年有101位藥師照顧4,032位個案，結果仍呈現門診就醫次數下降17％，門診醫療費用下降10％的結果，健保局給付藥師1元，藥師仍可下降門診醫療費用1元。藥師已準備承接更多病人的照護，而以保障民眾用藥安全與健康為最重要的專業使命。

In 2007, after the amendment of pharmacy law to add pharmaceutical care related activities in Article 15, pharmacist start by law to provide pharmaceutical care to needed patients. In 2009, the President of Taiwan Pharmacist Association successfully convinced the Bureau of National Health Insurance (BNHI) to set a budget of US$310,000 for trained pharmacists to conduct a pilot project in 2010: ”Improvement of medical seeking behaviors for high users of medical resources- home pharmaceutical care”. The first year project was conducted in north and south part of Taiwan, total of 8 cities, with 42 pharmacists took care of 808 individuals. The cost-benefit ratio was 1:3.3 for the first year. BNHI decided to continue the project in 2011 and expand to the whole island of Taiwan, 22 cities, with 4 times increase the budget. The second year, with 101 trained pharmacists took care of 4,032 individuals, the outpatient physician visits again decreased 17%, and outpatient medical expenditures decreased 10%, with a cost-benefit ratio of 1:1. Pharmacist in Taiwan is expanding their role in taking care of patients directly to ensure the safety and appropriateness of drug usage for all citizens in Taiwan.
