摘要

本文為探討一位複合性局部疼痛症候群（Complex regional pain syndrome, CRPS）病人以靜脈血管雷射處理疼痛之護理經驗。護理期間為101年3月7日至3月31日，運用Gordon十一項健康功能評估確立健康問題為：1.疼痛／骨骼肌肉神經痛2.與疾病治療相關之知識缺失／對靜脈血管雷射治療不了解3.自我照顧能力不足／進食、沐浴、如廁、移位需協助。個案形容罹患此症令其痛不欲生，筆者運用傾聽技巧，並透過跨領域間醫療合作，提供個別性護理，使個案疼痛指數（VAS）由7~9分降至3~4分，日常生活活動（Activities of daily living, ADL）亦由第三級進步到第一級。期盼此次照護經驗能提供其他護理人員深入了解此疾病，進而日後達到全人照護之目標。

This paper explores nursing experience in a patient with Complex Regional Pain Syndrome (CRPS), who received Intravascular Laser Irradiation of Blood (ILIB) for pain management. The nursing period was from March 7 to March. 31, 2012. The author applied Gordon 11-item Functional Health Pattern for nursing assessment. The patient’s health problems were (1) Pain, (2) Knowledge deficit, and (3) Self-care deficit. Pain is a very subjective feeling. The patient described it as unendurable agony. The author provided individual nursing interventions, including listening skills to help relieve pain through interdisciplinary medical team cooperation. At last, the patient’s pain index dropped from 7-9 points to 2-3 points by visual analog scale (VAS). Activities of daily living (ADL) in a patient with CRPS progress from the third level to the first level. The author provided this experience of adopting the effective treatment for pain, hoping that nursing staff can better recognize this disease and achieve holistic caring.
