摘要

近年來健康照護組織愈趨龐大，分工雖然精細，但各部門間傾向獨立作業，缺乏適當溝通；因此，健康照護組織的資源整合甚為重要。本文提出荷蘭社區服務新模式（Buurtzorg）與歐洲老年人的持續整合照護計畫（Sustainable Tailored Integrated Care for Older People in Europe，SUSTAIN）之簡介，提供台灣未來之政策參考；並且進一步討論社區整合照護模式未來應如何調整健康服務方向，以達到以人為本與社區共享價值的目標。

Over the last few years, health care organizations have grown larger and the work has become more divided. However, lacking proper communication channels between departments that operate independently still exists and there are always too many coaches but not enough players. Therefore, resource integration is essential but what is to be centered in integration is a question that worth considering. This paper presents a brief introduction to the new model of the Dutch Community Service (Buurtzorg) and the Sustainable Tailored Integrated Care for Older People in Europe (SUSTAIN) and provides a reference for the making of future policies. How to adjust the direction of health services in the community integrated care model and to achieve the goal of people-centered and community sharing value would worth further collaboration.
