Over the last few years, health care organizations have grown larger and the work has become more divided. However, lacking proper communication channels between departments that operate independently still exists and there are always too many coaches but not enough players. Therefore, resource integration is essential but what is to be centered in integration is a question that worth considering. This paper presents a brief introduction to the new model of the Dutch Community Service (Buurtzorg) and the Sustainable Tailored Integrated Care for Older People in Europe (SUSTAIN) and provides a reference for the making of future policies. How to adjust the direction of health services in the community integrated care model and to achieve the goal of people-centered and community sharing value would worth further collaboration.
