中文摘要 台灣的剖腹產率於2000年已昇至34.47%。剖腹產婦女面臨剖腹生產時的環境與聲音、身體不舒適及生理變化,會產生焦慮與壓力反應以及不愉快的生產經驗。音樂治療有助於個人身心靈的整合,臨床上可當作一種輔助療法,因此本研究旨在探討音樂治療是否能改善婦女在剖腹產過程的焦慮、壓力及剖腹生產經驗滿意度。本研究採實驗性研究法,於南部一所醫學中心的產房及手術室,以立意取樣選取64名預期性剖腹生產婦女,隨機分派為實驗組與對照組各32名。實驗組接受一般剖腹生產過程的照顧及至少30分鐘戴耳機的音樂治療,對照組只接受一般剖腹生產照護。研究工具包括焦慮視覺類比量表、壓力生理反應評估表及剖腹生產經驗滿意度評估表。重複量數雙因子變異數分析的結果,顯示音樂治療組比對照組能顯現改善婦女在剖腹生產過程的焦慮感受,並增進婦女對自己的健康狀況、對維持身體舒適感、對體驗手術過程的安全進行、對人時地完全了解和剖腹產術中環境的氣氛等剖腹生產經驗滿意度。音樂治療能改善剖腹生產婦女之指溫、動脈血氧飽和度、脈搏、血壓及呼吸,但未達統計上顯著差異。實驗組中81.2﹪對提供音樂表示「很滿意」。建議護理臨床推廣音樂治療,促進婦女在剖腹生產過程的身心舒適。 關鍵詞:音樂治療、剖腹生產、焦慮、滿意度
Abstract In 2000, the rate of Cesarean delivery has increased to 34.47% in Taiwan. Notably, women who underwent Cesarean section usually have suffered unexpected and unpleasant experiences due to physiological and psychological stresses. To date, music therapy has been applied to take care of patients for decreasing anxiety and enhancing health in a variety of fields and has showed promising results. The purpose of this experimental study was to investigate the effects of music therapy on women’s anxiety, stress and satisfaction during Cesarean delivery. The participants were women who had planned Cesarean delivery at a tertiary Medical Center in Southern Taiwan. In total, sixty-four women were randomly assigned to the experimental and the control groups. The experimental group received the traditional routine care and music therapy during the Cesarean section. The control group received the traditional routine care only. Instruments utilized in the study were the Visual Analogue Scale of Anxiety, the Physiological Indexes and the Satisfaction of Cesarean Delivery Scale. Our results indicated that the experimental group had significantly lower anxiety and higher satisfaction of Cesarean experiences (including perceived health status of herself, maintaining physical comfort, seeking safe passage for herself, ego orientation and birth atmosphere) than the control group. No significant differences were found in all physiological indexes between two groups. Of interest, eighty-one percent of the women assigned to the experimental group were very satisfied about the music therapy. In conclusion, we suggest the utilization of music therapy to promote the comforts of women during Cesarean delivery. Key words: music therapy, cesarean delivery, anxiety, satisfaction
