摘要
毛髮質瘤(pilomatrixoma)，是一種源自毛囊根部細胞的皮膚腫瘤，於任何年齡層均可發生，但以小孩子最常見，罹病率女性比男性高，無明顯種族差異。臨床上常以單一、硬、不痛、可動的結節表現。頭頸部為此腫瘤最好發的部位。毛髮質瘤可有惡性變化之表現，但並不常見，一般手術切除有良好的預後。本院於2002年7月經歷一38歲女性，主訴右耳有一無痛結節。回顧其病史，10年前在其同側耳前處亦有相同結節，而在幾個月前其右側下肢亦有相似病灶。患者無特殊疾病史，亦無家族病史。理學檢查方面，右耳耳輪上有一硬、無痛、可輕微移動、具灰白色內含物之結節，表面平滑有毛細血管擴張，大小約2cm。經手術移除病灶及部份表皮，病理切片診斷為毛髮質瘤。至今沒有發現局部復發或身體其它部位有類似病灶。

Pilomatrixoma is a skin neoplasm arising from hair follicles. The tumor can appear at any age, but most commonly occurs in children. No clear racial predispositions have been identified, but is seen in females more frequently. The tumor usually presents as a solitary, firm, painless and movable subcutaneous nodule. The head and neck regions are the most commonly affected. Malignant transformation of a pilomatrixoma has only rarely been reported in the literature, and recurrence after complete excision is relatively rare. A 38 year old female presented with a painless nodule affecting her right ear. After further history taking, it was found that the patient had suffered from a similar right pre-auricular lesion in the same location ten years previously, and had a similar lesion affecting her right lower limb a few months earlier. The patient had no symptoms of systemic disease or a significant family history. On physical examination, a firm, nontender, slightly movable subcutaneous nodule was palpable over the helix of the right ear and had gray-white contents. The surface of the nodule was smooth, around 2cm in diameter and telangetic. After complete excision of the tumor and partially adherent skin, the lesion was histopathologically confirmed to be a pilomatrixoma. No local recurrence or similar lesions elsewhere have been noted during followup to date.
