從實證觀點探討蒲公英外敷緩解產後婦女乳房脹痛之成效

摘要


背景:產後婦女於第二至四天常發生乳房腫脹,若未妥善處理,可能導致乳頭疼痛或破裂、乳汁供應減少、乳房炎症及母乳哺餵失敗。目的:基於傳統中醫理論,本文旨在以實證方法探討蒲公英外敷緩解產後婦女乳房脹痛之成效。方法:根據病人(產後婦女)、介入措施(蒲公英外敷)及結果(乳房腫脹或乳房疼痛)提出一個臨床問題。使用PubMed、Web of Science、EMBASE、Cochrane Library、MEDLINE、CINHAL與華藝線上圖書館學術網(CEPS及CJTD)等電子資料庫進行文獻檢索,以及Google學術搜尋是否有遺漏的已發表文章。進而,根據納入與排除條件,取得一篇隨機控制試驗文章。結果:蒲公英外敷聯合乳房按摩比單純乳房按摩可能對減少乳房腫脹消失時間及降低乳房疼痛更具有顯著成效。結論:蒲公英外敷聯合乳房按摩可能緩解乳房腫脹及乳房疼痛,然而,受限於納入文章篇數及研究方法學,建議可採行方法學嚴謹的隨機控制試驗,以提升試驗之價值性與可信度。

Background: Breast engorgement generally occurs between the second and fourth day after delivery in postpartum women. If not properly treated, it can lead to sore or cracked nipples, reduced milk supply, breast inflammation, and breastfeeding failure. Purpose: According to the theory of traditional Chinese medicine, this article aimed to explore the effect of external application of Taraxacum on relieving breast engorgement pain in postpartum women through an evidence-based approach. Methods: Asking a clinical question based on the patient (postpartum woman), intervention (external application of Taraxacum), and outcome (breast engorgement or breast pain). Electronic databases for literature search included PubMed, Web of Science, EMBASE, Cochrane Library, MEDLINE, CINHAL, and Airiti Library (CEPS and CJTD), and Google Scholar was searched for ascertaining any missing published articles. Consequently, one randomized controlled trial article was obtained according to the inclusion and exclusion criteria. Results: External application of Taraxacum combined with breast massage could have a more significant effect on shortening the duration of breast engorgement and relieving breast pain as compared with breast massage alone. Conclusion: External application of Taraxacum combined with breast massage could relieve breast engorgement and breast pain. However, because of the limited number of included articles and insufficient research methodology, conducting methodologically rigorous randomized controlled trials for enhancing the value and reliability of the trials is suggested.
