提升放射診斷科之醫療儀器請修作業效能:精實醫療

Improve the Performance of Medical Device in Radiology Department: Lean Healthcare

摘要


當醫院的醫療儀器出現問題時。工作人員首先必須先執行儀器的初始測試和檢查記錄,還需要聯繫醫院的醫學工程師及機器製造商的工程師。除此之外,他們還需要進入企業資訊入口網站(Enterprise Information Portal;EIP),填寫醫療儀器維修程序。事實上,工作人員不斷重複再做相同的事情。本研究主要目的是藉由精實醫療管理方法,消除不必要的醫療儀器請修作業的時間浪費和動作。2017年1月至12月,共收集了167份放射診斷科的醫療儀器維修單,進行了回顧性的分析。並使用「Google表單」系統,設計了問卷調查及滿意度表單。運用親和圖彙整開放性問題,親和出五大需求。接著利用實際觀察方法,觀察儀器請修之流程,包含了所有移動、時間與動作,繪製成現況價值流圖。最後再經由親和圖與現況價值流圖,找出六大問題點。流程改善後,於2018年12月期間分析20件機器請修件數。儀器請修作業時間,由35.3分降為9.6分。2.病人抱怨件數,由5件降為0件。滿意度調查,由2.92分提升至4.85分。精實醫療可減少醫療儀器請修作業的時間浪費及不必要的動作。運用其徹底消除浪費,讓醫療工作人員可以專注在照護上,改善病人醫療照護品質。

When there is a problem with the hospital's medical device. The staffs must first perform initial testing and inspection records of the instrument. In fact, the staffs are constantly repeating the same thing. This is equivalent to taking twice as long. The purpose of this study is to eliminate waste and unnecessary movement by lean management system. 167 radio medical device repair forms between January to December 2017 were retrospectively analyzed. Using "Google Forms" to make custom forms for surveys and questionnaires. Use the affinity diagram method to classify the five major needs of the staffs (radiologist). We observed the process of repairing the medical device, including movement, time and work. Create a value stream mapping of your current process. Six key points from the affinity diagram method and value stream mapping. After improving the process, each of the 20 repaired medical devices was analyzed in December 2018. The repair time for the medical device is reduced from 35.3 to 9.6. The number of complaints that our department receive is reduced from 5 to 0. The patient satisfaction increased from 2.92 to 4.85. Lean healthcare eliminates the waste of time, energy, or money by identifying each step in a business process. Then modify or eliminate the steps that do not create value and improve work efficiency. Eliminate waste and medical staff to the delivery of high-quality care to patients. And improve health care quality for patients.
