Endoscopic Treatment of Acute Lower Gastrointestinal Bleeding Caused by a Sigmoid Inflammatory Fibroid Polyp: Report of a Case

摘要

炎性纖維息肉是一少見的良性胃腸道息肉，此息肉好發於胃及小腸，大腸的炎性纖維息肉則相當罕見。此息肉源自於胃腸道壁的黏膜下層，組織學下可見富含纖維母細胞、血管組織以及許多的發炎細胞浸潤，特別是嗜伊紅性白血球。而其症狀取決於息肉所在位置及其大小型態，病人常以腹痛、腸阻塞、貧血、大便習慣改變或血便來表現。雖然血便在大腸炎性纖維息肉相當常見，但造成嚴重下消化道出血則非常稀少，而且只有少數案例是用內視鏡切除。我們報告一位以嚴重下消化道出血爲表現的乙狀結腸炎性纖維息肉，病患接受內視鏡檢查並接受息肉切除術後順利止血。

Inflammatory fibroid polyp (IFP) is an uncommon benign localized lesion of the gastrointestinal tract and occurs more frequently in the stomach. The small intestine is the next frequent site and they are rarely described in the colon. Histologically, it arises in the submucosa and characterized by distinctively arranged fibrous tissue and blood vessels, with an inflammatory infiltrate often dominated by eosinophils. The clinical presentation of IFP depends on location and gross appearance of the lesion. It included incidental finding, vague abdominal discomfort, obstruction symptoms, bowel habit change, anemia or bloody stool. Although bloody stool is a common presentation of IFP, life-threatening lower gastrointestinal bleeding due to IFP is rare and only few cases were resected endoscopically. We herein report a patient with an IFP localized in the sigmoid colon presenting clinically as life-threatening lower gastrointestinal bleeding and was successfully treated by endoscopic polypectomy.
