摘要


本篇報告一位三十六歲女性患者,右眼因爆炸傷害而造成角膜瘻管,經使用治療性軟性隱型眼鏡(Bandage contact lens)及配合其他相關的輔助治療,結果令人滿意。追踪一年六個月之後,其患眼視力一直保特在6/7.5。

A corneal fistula is a chronic, established communication between the anterior chamber and the corneal surface. An open fistula may be defined as a track through which aqueous humor has free access to the corneal surface and tear film. A closed fistula is similarly a track to the surface of the cornea, but it is closed at the external end and there is no access to the tear film. Corneal fistulas have been described as a rare complication of keratoplasty, cataract extraction, corneal ulcer, poor resuturing of corneal wounds, Terrien's marginal degeneration and keratoconus with hydrops. In all of these diverse circumstances, there was a perforation site that would not heal because of the combination of poor tissue apposition and persistent aqueous humor flow. Therapy of corneal fistulas including ocular hypotensive agents, bandage contact lens, chemical cauterization, diathermy, cryopexy, thermo-keratoplasty, conjunctival flap, and lamellar or penetrating keratoplasty. 36 year-old female, a case of open type corneal fistula which was resulted from an explosive accident, was treated with bandage contact lens and topical antibiotics in the presence of positive Seidel test result. The case was followed up for 18 months. The vision still maintain 6/7.5 and was to the satisfaction to the patient subjectively.
