在台灣以球後注射Chlorpromazine治療疼痛性盲眼之臨床經驗-六例病例報告

Retrobulbar Chlorpromazine Injections for Managing Blind Painful Eyes in Taiwan-Report of Six Cases

摘要


目的:報告以球後注射Chlorpromazine來治療疼痛性盲眼的臨床效果以及併發症。 方法:本篇採前瞻性方法研究自2002年10月至2003年11月於本院門診病人中連續選取符合條件的病人。共有六位病人的6隻眼睛接受我們的臨床治療。每一位病人皆至少接受三個月以上的追蹤。治療結果的評估主要為:術後需不需要口服止痛藥的幫助,疼痛程度的評估,眼壓、視力的檢測以及術後的併發症。 結果:所有六位病人的6隻眼睛在術後第二天都獲得有效的疼痛控制,而且所有病人在門診追蹤三到十二個月的期間中,並沒有疼痛復發的情形。在六位病人中有五位病人眼壓沒有明顯的變化,只有一位在術後一星期時眼壓明顯下降(由56mmHg降至25mmHg);視力方面所有病人都沒有改變。此外,在每一位病人的身上也沒有發生永久性的併發症。 結論:以球後注射Chlorpromazine來治療疼痛性盲眼,是一種有效、便宜、簡單而且不必住院的治療方法。

Purpose: To report the efficacy and complications of retrobulbar Chlorpromazine injection for blind painful eye management in Taiwan. Subjects and Methods: A prospective, uncontrolled before-after study enrolled six eyes of patients with minimum three months of follow-up. In each case, 3-4 ml 2% Xylocaine hydrochloride subsequent 1.5 ml (25mg/ml) Chlorpromazine were injected into the retrobulbar space of blind painful eye. Intraocular pressure (IOP) and pain scale were recorded before and after the procedure. Results: All six patients felt pain relief following this procedure immediately. None complained of previously experienced ocular pain symptom in subsequent follow-up. No significant IOP reduction was noted in five of 6 patients after the injection. Only one patient got 50.6% IOP reduction one week after treatment. Post procedural complications of eyelid swelling and chemosis were noted in all patients, but all resolved within a few days. Limitation of ocular movement that persisted for four weeks was noted in one patient. Conclusion: Retrobulbar Chlorpromazine injection appears to be a safe and effective procedure for blind painful eye management.
