病人影像與資料的數位化在長庚醫院的經驗

摘要

影像在皮膚科是診斷非常重要的依據，以往為了將病人的資料存檔，大部分的醫師是使用單眼相機記錄病人皮膚的情況，再用照片或幻燈片來整理，這樣的方式相當耗時與費工。隨著數位相機的盛行與高階機種的推出，加上電腦與網路的便利，影像資料的整理與儲存有了更多的選擇。台北長庚醫院在過去的一年半建立了一套數位化系統，使用數位相機配合單眼相機來幫病人拍照，然後以資料庫軟體來整理病人的照片、資料與病理診斷，並使用院內網路將林口，台北和基隆院區的資料彙總作整理，最後再使用光碟來備份，這樣的方式不僅節省了時間與金錢，而且能使病人的追蹤與病例的收集更完整：同時我們也能將這些影像資料提供給病理醫師，使病理醫師在看病理切片的同時也能看到病人的臨床照片，讓病理診斷能夠與臨床配合，而不只是靠著臨床醫師的文字描述來想像臨床的皮膚變化。雖然影像資料的數位化與網路化有很多的好處，但相對的還有一些問題要克服，像是數位照片的品質，網路的安全性等等，但相信這一定是未來的趨勢。我們將台北長庚醫院這一年的經驗分享出來，並希望大家在將來能建立一共同資料庫，讓皮膚影像資料能夠相互流通。

Dermatologist relies much on photograph images. Thus, saving and filing photograph images are very important. Traditionally, we take pictures of patient’s skin lesions with SLR (single light reflex) cameras and then save them as slides or photographs, which require considerable time and effort. However, as more functions of the digital cameras have been developed and as computers and Internet become more popular, we have more options. During this past year, we have been taking patients’ pictures by using digital cameras and SLR. Furthermore, we have been filing patients’ data and pathological diagnosis using database software. At the same time we have been collecting data from Linkou, Taipei, and Keelung via intranet and stored them with compact disks. Digitalization of these data not only save time and money, but also make patients’ follow-up and case collection more convenient and less time-consuming. Meanwhile, we also offered patient’s data and pictures to the pathologists so that they are able to make the diagnosis not just by depending on dermatologists’ literal statements. Although there are many benefits of digitalization of patients’ images and data, we still have to overcome some problems, such as quality of the pictures, safety of Internet…etc. Digitalizing data will become the mainstream in our field.
