運用人因工程改善客觀結構式臨床測驗作業流程之專案

An Improvement Project of Objective Structured Clinical Examination Operation with Adoption of Ergonomics

摘要


客觀結構式臨床測驗(簡稱OSCE)為提供結構且安全的臨床模擬情境,以提升新進人員護理能力及護理教師考官教學能力之雙重成效,因此OSCE作業流程管理相當重要。在教學過程中,發現考官對衛材擺放位置不熟悉及未明確標示位置、共識會議訊息傳達不完整,以致對OSCE作業流程滿意度3.28分、教學意願51.2%及衛材溢領9,768元。改善方案,與跨部門合作衛材空間規劃、整合OSCE考官共識作業、建置OSCE標準作業指引、建構OSCE衛材E化管理。介入後滿意度提升至4.52分,教學意願提升至81.4%,衛材溢領量下降至0元,達到改善目的。專案結果,將繁瑣OSCE作業透過跨團隊協調合作,整合考官共識,運用人因工程概念建置標準作業指引及資訊科技導入落實衛材管理,提升OSCE過程教與學之品質。

Objective Structured Clinical Examination (OSCE) is a safe and structured way of simulation teaching methods in clinical circumstances to improve the teaching skill of the examiner and the performance of the newly employed nursing personnel. Consequently, the procedural management of OSCE is very crucial. During the teaching practice, the author discovered the location of the medical materials were not well marked, making the examiner become less familiar with where they were put, as well as the consensus meeting message being incomplete. As a result, the satisfaction of examiners with OSCE procedure was only graded at 3.28 points, the teaching willingness of the preceptor is 51.2%, and the abnormal usage of medical material cost 9,768 NTD. To make better improvement, a cross-departmental cooperation on a spatial plan for medical materials was set up. The common operating procedure from OSCE examiners was then integrated. A guide for OSCE procedural standards and the computerized OSCE material management was then established. After implementing the improvement project, satisfaction was increased to 4.52 points, the teaching willingness of the preceptor increased to 81.4%, and there was no cost overrun for medical material charge. The implementation of the project indicated the strenuous OSCE operation had been improved through cross-group collaboration, the integrated consensus of clinical preceptors in nursing, the use of ergonomics-oriented guidance for the establishment of standard operating guidelines, and the application of information technology for managing medical material. This enhanced the quality of both OSCE process teaching and learning.
