探討青少年精神患者母親之生活經驗

本研究之目的在了解當家中出現青少年精神病友時，主要照顧者的母親之心路歷程與轉折。研究之對象取自於南部某區域教學醫院精神科，經醫師診斷具認知功能障礙或情感疾患之12-20歲青少年的母親，共有6位平均年齡46歲的受訪者參與此研究。依循研究步驟，以深度訪談收集資料並透過訪談過程全程錄音，完整記錄母親的生活經驗。訪談的資料運用主題分析法進行資料分析，共呈現五個主題，『尋找歸因』、『遭遇困境』、『角色拉扯』、『形成結盟』及『轉變境域』。本研究顯示：首先母親由孩子外顯行為或個性改變的異常為出發點尋找到「超自然」、「他人與社會關係/壓力」及「體質/遺傳」等歸因，而在陪伴過程中母親會遭遇「不安定感」、「經濟負擔」、「不被理解的心情」及「他人批判的眼光」等困境。也會面臨角色拉扯的「分身乏術」，並且因為「被怪罪/罪惡感」及「難放棄母親角色」，因而與異常的孩子形成孤獨的結盟。但母親透過轉念，「認清現實的無法度，尋求突破」、「找到平衡相處之道」、「接受宿命的安排」，並「依循孩子的特性去協助」，不斷轉變境域，最終找到自己的平衡。此研究增加對青少年精神病友母親的生命經驗之了解，有助於臨床的護理人員與母親互動時更貼切同理心的應用。關鍵字：青少年精神病、母親的生活經驗、質性研究

The study is taken from a southern regional teaching hospital psychiatry. The informants were six mothers, whose Adolescent children have been diagnosed as Schizophrenia or Mood disorder. After using in-depth interview with recording pen, the datas were transcribed into texts to explore participate mothers’ lived experience completely. The interview texts were analyzed by consistent content analysis method. From the findings of this study, Five major themes indicated their concerns. Looking for attribution, Encountered Dilemma, Forming a mother-child dyad, Roles pulling, and Transform the territorial boundaries. The findings reveal that：Through explicit behavior or personality changes, abnormal as a starting point to look for attribution, including paranormal, social relations with others/pressure and Physique/genetic. Mothers encountered all kinds of Dilemma in the process of companionship, like uncertainty, Economic worries, Feelings not be understood, and Others’ critical eye. The mothers also faces Roles pulling, blamed/guilt and Difficult to give up the role of mother, and then forming a loney dayd with the abnormal children. But eventually they will transform the territorial boundaries and find their own balance. The study will help nurses understand the mothers’ lived experience.
