Cystic pheochromocytoma is an unusual variant of pheochromocytomas, most of which have been reported as clinically silent. A case of cystic pheochromocytoma with initial presentation of acute myocardial ischemia was subsequently found in a suprarenal cystic mass by sonography. Magnetic resonance (MR) imaging and computed tomography (CT) showed a large suprarenal cystic tumor with hemorrhagic fluid layering. The MR and CT imaging of this case is discussed.
