Fracture of the entire posterior process of the talus is rare and when it does occur, is often associated with significant morbidity as the fracture typically involves two articular surfaces, and compresses the posterior tibial neurovascular bundle. We report on a case of fracture of the entire posterior process of the talus together with a depressed subtalar articular fragment, which was treated with elevation of the depressed fragment and support of the upper void with OsteoSet. At follow-up, the patient recovered well and obtained a good functional outcome.
