One case of basal cell carcinoma of the inner one third of the lower eye-lid (OS) is presented. The tumor (6×5×4 mm) invaded lid margin and full thickness of the eve-lid. If the tumor were to be excised by frozen section control, then the canaliculus and the punctum should have been excised with consequent epiphora. Therefore, the actual operation involved excision of the carcinoma to its 2 mm surrounding, lid-reconstruction and cryotherapy of the lower eye-lid. Up to the present time, (8 months after the operation) no recurrence/metastasis is noted. The cosmetic appearance is satisfactory. And no epiphora has occurred. The frozen section control technique stresses the advantage of no recurrence, hence extensive excision is unavoidable. And the lacrimal passage integrity and cosmetic problem are less threatened. According to the author's opinion, no recurrence of course is the highest priority, but the integrity of the lacrimal passage system and the post-operative cosmetic appearance should also be considered.
