汽車擋風玻璃造成的眼破裂傷分析

Penetrating Ocular Injuries Due to Windshields

本篇用回溯法分析長庚紀念醫院自民國80年1月至82年2月共二年二個月期間,因汽車擋風玻璃破裂造成眼破裂傷,接受手術治療之病例共30例(32眼),其中男性25例,女性僅有5例;年齡自8歲至57歲,以31~40歲最多(33.3%),41~50歲次之(26.7%),所有病例所乘坐車輛皆使用一般強化玻璃(非膠合安全玻璃),手術後經至少三個月追蹤,視力恢復至0.1以上達18眼(56.3%),視力恢復達0.5以上有12例(37.5%),14眼視力不能恢復至0.1,主要原因為玻璃體機化及視網膜剝離(各佔5例),破裂傷依傷害程度分為五級,視力預後與傷害等級有關,對於汽車擋風玻璃破裂導致的視力損傷,汽車全面使用安全膠合玻璃做事前的預防,勝於事後治療。

A retrospective study was undertaken to review 30 patients (32 eyes) who underwent primary surgical repair of penetrating ocular injuries caused by broken windshields at the Chang Gung Memorial Hospital from January 1991 through February 1993. Among these patients, there are 25 males and 5 females with age ranging from 8 to 57 years. The group of age at 31 to 40 years ranked highest number (33.3%). None of the windshields of the accident cars is composed of shatter-proof glass. Examination of the recovery of vision at least three months after the injuries revealed that more than half of the cases (56.3%) recovered to a vision better than 0.1, 37.5% better than 0.5. However, 14 eyes remained at a vision worse than 0.1; vitrous organization and retinal detachment were the major causes. The degree of vision recovery depends on the severity of the injuries. Further enforcement of safety precautions is indicated, in particular the use of shatter-proof glass which should be freely available.
