視神經盤小孔併非裂孔性網膜剝離-病例報告

摘要


視神經盤小孔與漿液性網膜剝離間之密切關係已是廣為人知。有關其疾病生理學及造成網膜剝離之機制,雖有各種理論曾被提出,但並無確切之結論。我們報告一個視神經盤小孔併漿液性網膜剝離之病例,患者在經過玻璃體切除、氣液交換術、及一系列雷射治療後,雖於其間有二次網膜再次剝離,但最終網膜成功地貼回復原,視力由辨指數進步至零點零叁。經過七個月之追踪,網膜穩定無再次剝離之現象。術中我們試圖由視神經盤小孔以真空吸取網膜下液體,雖無法完全吸除,但由網膜下液體之減少,我們推論,在有關網膜下液體來源之諸多理論中,以網膜下空間經山視神經盤小孔與玻璃體腔之間有直接連繫之理論,於此病例中,是最為可能的。

Various theories of pathophysiology about serous detachment on optic disc pits had been speculated. We reported our experience of a case presenting with nonrhegmatogenous retinal detachment limited within the posterior pole. The retina was attached after vitrectomy and gas tamponade with internal drainage directly through the disc pit. However, serial sections of fluid gas exchange and laser photocoagulation were required to stabilize the recurrent detached retina. We concluded that the theory of direct communication between subretinal space and the vitreous space via the optic pit is favored in this case.
