由巨細胞病毒視網膜炎診斷出愛滋病－病例報告
Cytomegalovirus Retinitis as the First Clinical Sign in the Acquired Immune Deficiency Syndrome-A Case Report
摘要
33歲男同性戀者，於民國84年4月份感到左眼視力逐漸糢糊，至地區眼科診所求診，診斷為左眼眼底視網膜血管阻塞(Branch retinal vein occlusion)及視網膜出血，而接受眼底雷射治療，治療三個月後，視力並無改善，病人於民國84年9月份，感到左眼視力明顯減退，而轉診至本科求診。在高度懷疑巨細胞病毒視網膜炎(Cytomegalovirus retinitis)的診斷下，抽血檢驗證實病人感染HIV(+)。 近年來向於台灣地區愛滋病患者不斷的增加，在提出此一罕見病例，以提醒醫界同仁，眼科醫師將可能是愛滋病第一線的發現者。

Cytomegalovirus (CMV) retinitis is the most common ocular infection in patients with the acquired immunodeficiency syndrome (AIDS). CMV retinitis usually develops in the late clinical stage of AIDS patients whose CD4(superscript +) counts are less than 50/mm^3 whereas CMV retinitis, as the initial manifestation of AIDS, is seldom reported. We report a homosexual man infected with CMV retinitis which is leading to diagnosis of AIDS, and high CD4(superscript +) counts (134/mm^3) was also found.
