A High Angle Skeletal Class Ⅱ Malocclusion Treated with Orthognathic Surgery and Canine/Incisor Substitution

以正顎手術及非典型拔牙治療骨性Ⅱ級咬合不正之病例報告

摘要

本篇報告一女性年輕患者為長臉型骨性Ⅱ級咬合不正之治療過程。此病患有外傷病史及顳顎關節疾患，其雙側正中門齒亦因外傷治療預後不佳而計劃拔除。為達成外觀和咬合功能之改善並避免拔除額外的上顎牙齒來改善上顎前突，遂於正顎手術之設計中儘量利用此門齒縫隙來改善，並避免大範圍之下顎手術以防加重顳顎關節疾患。術後以齒顎矯正配合補綴方式以側門齒代替中門齒，以犬齒代替側門齒。本文將對此治療計劃，手術設計，及非典型拔牙於美觀與功能的考慮加以探討。

A case of a young female with traumatized central incisors, skeletal Class Ⅱ malocclusion and vertical excess with retruded mandible was treated with orthognathic surgery and atypical extraction by canine/incisor substitution. The treatment considerations of surgical design and finishing of the esthetics and occlusion of atypical extraction were discussed.
